What is your fee?

My fee is $140 for a 53 minute session. I accept cash, credit, or check. Payment is expected at the time of service. Please see my disclosure form for further information.
 

Do you take insurance?

I am a contracted provider for the following insurance plans: 

  • First Choice Health
  • Kaiser (PPO plans)
  • LifeWise
  • Premera (and out of State Blue Cross Blue Shield plans)

I think you would be a good fit, but I don't see my insurance plan listed.

I encourage you to call the number on the back of your insurance card to find out what your out of networks benefits are. Using your out of network benefits means that you are responsible for fee at the time of service, but I can provide you with the appropriate documentation that you can submit to your insurance company for possible reimbursement. 

 

WHERE IS YOUR OFFICE? IS IT ACCESSIBLE?

My office is in Mountlake Terrace, about five blocks away from I-5 and the Mountlake Terrace Transit Center. Click here for a map. The office has a designated parking lot and is ADA accessible.
 

What do I need to bring to my first Appointment?

I have a couple forms that I use for my intake process. You can click here if you would like to print and fill them out ahead of time. I will also have them available when you arrive for your first appointment. Please arrive 10-15 minutes before your appointment so that you have time to fill them out. If you choose to use your insurance, also bring your insurance card.
